Contacting the Palliative Care Service

In addition to the CCPC Inpatient Unit, there are palliative care teams onsite at Concord Hospital, Royal Prince Alfred, Canterbury Hospital and a team that supports people in the community.

Palliative care referrals are made when extra attention to worrying symptoms or closer follow up at home is needed.  To be referred to the Palliative Care Team you need a referral from your specialist or GP. For more information about our palliative care services phone:
